This is Magic's "Ultimate Game Jersey" card from the 2003-04 Ultimate Collection series. It features a beautiful game-worn jersey swatch (not just player-worn but game-worn). A hard to find card for Magic as it makes for awesome memorabilia, especially from that era.
It is also short print with only 200 ever made. This card is numbered 113/200.
